Hello and welcome to the Rail Safety in Australia mailing list.
With the expansion and diversification of rail transport over time, issues relating to safety become more and more paramount. Rail operators are always treading a fine line between economic viability and costs of operating in a manner which is as safe as possible, and trade-off's are always being made to meet operational requirements. It's not just a problem that affects government-owned rail operators.
